RN Hospice Case Manager
Interim HealthCare is seeking a compassionate and skilled Registered Nurse (RN) to join our team immediately! The need is for a Hospice Case Manager to work full time and can start immediately.
The nurse will meet patients both in a Hospice facility in StPaul as well as in their homes within a 25 mile radius of the facility.
Most staff start and end their day in the office to gather/ return supplies and connect with supervisor.
Schedule:
M-F 8-4 or 9-5.
Some days could be a bit shorter, some a bit longerHourly pay rate:
$65/ Hour plus mileage from client to client
Responsibilities:
Conduct comprehensive health assessments for residents, including vital signs, medication management, and overall health status monitoring.
Collaborate with the interdisciplinary team and families to develop and update individualized care plans.
Administer medications, ensure proper documentation, and educate patients and their families about medication regimes.
Provide health education to patients and their families.
Coordinate and communicate with other healthcare professionals, including physicians, therapists, and support staff, to ensure comprehensive and holistic care for residents.
Respond to medical emergencies and deaths promptly, efficiently, and compassionately following established protocols and providing immediate care as needed.
Maintain accurate and up-to-date medical records, documenting all assessments, interventions, and communications related to resident care.

Ensure compliance with state regulations as well facility policies, and procedures.
Carry a work cell phone during working hours.
No on call responsibilities.
Requirements:
ADN degree in nursing in nursing from an accredited school.
Current and valid RN license in the state of Minnesota.
Valid drivers license and vehicle to drive from client to client Experience with Hospice or Homecare is preferred.
Flexibility with schedule as some days could be shorter and some could be longer.
There is the possibility of overtime.
